The Champaign County Coroner's Office has identified an Avon, IN man who died from injuries in a two-vehicle crash last Thursday. 19-year-old Andrew Wade Thomas died at Carle Hospital in UJrbana about 30 minutes after the crash.  The accident happened at the intersection of University Avenue and 4th Street around 10 pm.  

Champaign Police reported the victim was thrown from his cycle after colliding with a car.  Police reported a westbound car failed to yield in making a left turn onto the 4th Street.  Police ask anyone with info about thge accident to contact polcie at 217-351-4545.
